Access Records from Before February 1, 2022

To access the records from before February 1, you can save or print your entire record from your old patient portal (My ezAccess) by following these steps:

Log onto the ezAccess patient portal:

1. From the left navigation: Select Documents

2. For each patient you have access to: Select the Patient Name in the Patient selection box

• At the top of the page, select View Medical Information

• Select the “eye” icon on the far-right side of the view

• Medical Data: Default is Include All, Select View

• Message displays: Processing request…

• The CONTINUITY OF CARE DOCUMENT will display

• Select Download - HTML
